About the Role

This role involves identifying the needs of young people by delivering targeted support that focuses on their genuine aspirations and goals.

You will be directly assisting young people to access a range of services to achieve positive behavioural changes and engagement with Youth Case Managers.

Duties

  • Engage positively and warmly with young people to encourage and guide them to achieve their objectives.
  • Deliver comprehensive assessments with young people to determine their genuine needs and aspirations.
  • Provide targeted supports to a caseload assessed as low and/or moderate, and high needs young people, aiming to achieve their desired outcomes.
  • Coordinate access to specialist services and provide direct support.
  • Maintain quality systems, prepare data, and reports.
