Empowering Lives through Care Coordination

A care coordinator is sought to enhance the lives of older Australians by joining our team. This role focuses on empowering individuals to maintain their home comforts, health, and connections within their community.

About the Role

This permanent full-time position is based in a regional office with hybrid work-from-home arrangements and travel to clients' homes in designated areas when required.

Key Responsibilities

  • Conduct comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders.
  • Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visits.
  • Collaborate effectively with internal stakeholders to ensure clients' needs are met, including our Rostering team and Practice Leads.
  • Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities.
  • Represent the organisation at external forums and meetings, promoting our values within the community.

Skills & Experience Required

  • Demonstrated experience in aged care or allied health case management with strong administration skills, and ability to work with individuals, families, and/or stakeholders.
  • Understanding of Home Care Packages, Commonwealth Home Support Programme, Aged Care services, and My Aged Care Standards.
  • High-level competency in utilising a variety of technology, including mobile devices and computers, proficiency with Microsoft Office, Outlook, and database systems.
  • Ability to identify and implement risk mitigation strategies.
  • High-level communication, influencing, liaison, and negotiation skills, established problem-solving and conflict management skills.
  • Ability to multi-task and work autonomously in a fast-paced environment.
  • A current unrestricted Australian Driver Licence, reliable vehicle, and a willingness to travel for work purposes.
