Position Overview:We are seeking a dedicated and empathetic Homelessness Case Manager to join our team to provide personalized support to individuals and families experiencing homelessness. The ideal candidate will have a passion for social justice and experience in working with vulnerable populations.
Key Responsibilities:- Client Support: Conduct intake assessments and develop individualized service plans for clients. Provide one-on-one support and counseling to help clients achieve their goals.
- Resource Navigation: Assist clients in accessing community resources, including housing, healthcare, employment services, and financial assistance.
- Advocacy: Advocate on behalf of clients to ensure they receive necessary services and support. Work with community partners and agencies to address barriers and challenges.
- Documentation: Maintain accurate and up-to-date case records, including progress notes, service plans, and outcomes. Ensure compliance with organizational policies and funding requirements.
- Crisis Intervention: Provide immediate support and intervention in crisis situations. Develop and implement safety plans as needed.
- Program Development: Contribute to the development and improvement of programs and services based on client needs and feedback.
- Education: Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
- Experience: Minimum of 2 years of experience working with individuals experiencing homelessness or similar vulnerable populations.
- Skills: Strong interpersonal and communication skills. Ability to build trust and rapport with clients from diverse backgrounds.
- Knowledge: Understanding of the challenges faced by people experiencing homelessness and familiarity with community resources and services.
