Job Title: Case Manager

UnitingSA is seeking a skilled Case Manager to join our Homelessness team. As a Case Manager, you will provide intensive support to individuals and families experiencing homelessness, working closely with clients to develop and implement personalized case plans.

Key Responsibilities:

  • Deliver case management services aligned with UnitingSA's framework for homelessness.
  • Offer goal-oriented services to meet client and customer needs.
  • Manage referrals, assess needs, and provide appropriate services.
  • Respond to client enquiries and address complaints.
  • Facilitate linkages and referrals to other agencies when needed.
  • Identify priorities and manage high workloads to meet deadlines.
  • Execute engaging programs for relevant service outcomes.
  • Provide information and referral pathways for self-management support.

Requirements:

  • Hold a relevant Human Services qualification or equivalent degree.
  • Proven experience in diverse intervention models within human services.
  • Skilled in crafting detailed reports.
  • Expertise in case planning and complex case management for marginalized individuals.
  • Hands-on Case Management experience, covering assessment, planning, referral, and advocacy.
