About the Role:

As a Support Coordinator, you will play a vital role in empowering individuals with disabilities and their families or carers to achieve their goals and enhance their quality of life.

You will utilise strengths-based practice to support clients in identifying their needs, preferences, and goals, promoting their independence and participation in the community.

The ideal candidate will possess relevant qualifications and/or experience working within the Disability sector, demonstrating a strong understanding of the needs of people with disabilities and their carers.

You will work collaboratively to deliver services that cater to the unique needs of each individual, ensuring effective monitoring and reporting through accurate and relevant client records.

Additionally, you will provide information and referrals to support clients in accessing appropriate resources and services, both within the organisation and externally.

Your Key Responsibilities:

  • Support clients to develop, implement, and monitor personal goals
  • Establish and maintain relationships with key agencies and service providers
  • Ensure accurate and relevant client records are maintained
