Role Overview

We are seeking a Clinical Care Coordinator to provide clinical care coordination and mental health support for clients with complex needs.

About the Position

In this role, you will deliver short-term, goal-oriented case management and brief interventions to help reduce barriers to care and improve access to mental health support. You will engage with clients to address psychosocial needs and improve mental health outcomes, collaborate with internal and external stakeholders to support recovery-focused care, contribute to group programs and quality assurance initiatives, maintain accurate client records, and participate in clinical supervision and ongoing training.

Key Requirements

The successful candidate will be a Psychologist, Occupational Therapist or Registered Nurse with full registration with AHPRA, or a Social Worker with AASW membership, or hold a Master's Degree in Counselling and registration with PACFA (Clinical) or ACA (Level 3/4). You must also have the ability to obtain a Working with Vulnerable People/Children Check, Australian Citizenship, and eligibility for Baseline Clearance.

Contract Details

This is a 12-month contract with potential extensions, offering a competitive hourly rate of $55-$65 + superannuation.
