Aged Care Services Coordinator Position

Salary:
A competitive salary package is offered, including options for salary packaging up to $18,900 net per annum.
Job Description:
The successful candidate will play a vital role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients. Reporting to the Clinical Care Manager of Home and Community Care Services NSW, you will need:
- Candidates with a Certificate IV in Individual Support and case management experience or an Enrolled nurse qualification are encouraged to apply.
- The ability to balance client expectations with available resources, effectively communicating these to clients.
- Develop and review individualised care plans based on client assessments and preferences.
- Excellent communication, interpersonal, and negotiation abilities.
- Maintain accurate records and documentation in line with organisational policies and government funded guidelines.
- A collaborative approach to working as part of a team.
- Knowledge of Commonwealth Support Programme (CHSP) is highly valued.
- Demonstrated experience in assessment, referral, and coordination of human services.
- An unrestricted driver's license.
This role requires travel to client's home however office based tasks can be completed remotely (WFH).
Required Skills and Qualifications:
- Certificate IV in Individual Support and case management experience or Enrolled nurse qualification.
- Ability to balance client expectations with available resources.
- Individualised care plan development and review skills.
- Effective communication and interpersonal skills.
- Accurate record keeping and documentation skills.
- Collaborative team work approach.
