About the Role
At McLean Care, we are seeking a skilled Care Coordinator to join our team in the Hunter Region. As a Care Coordinator, you will play a vital role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients.
Key Responsibilities
- Deliver exceptional care services through assessment, case management, and service monitoring.
- Develop and review individualised care plans based on client assessments and preferences.
- Effectively communicate with clients, balancing their expectations with available resources.
- Maintain accurate records and documentation in line with organisational policies and government funded guidelines.
- Work collaboratively as part of a team to achieve quality care outcomes.

Eligibility
