The Role

As a Care Coordinator, you will play a vital role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients, aligned with McLean Care's values and policies.

  • Candidates with a Certificate III in Individual Support and case management experience or an Enrolled nurse qualification are encouraged to apply.
  • The ability to balance client expectations with available resources, effectively communicating these to clients.
  • Develop and review individualised care plans based on client assessments and preferences.
  • Excellent communication, interpersonal, and negotiation abilities.
  • Maintain accurate records and documentation in line with organisational policies and government funded guidelines.
  • A collaborative approach to working as part of a team.
  • Knowledge of Commonwealth Support Programme (CHSP) is highly valued.
  • Demonstrated experience in assessment, referral, and coordination of human services.
  • An unrestricted driver's license.

Eligibility
