Care Coordinator

About the Role:
The Care Coordinator will play a vital part in delivering exceptional services through assessment, case management, and service monitoring for eligible clients, aligned with McLean Care's values and policies.
Key Responsibilities:
- Develop and review individualised care plans based on client assessments and preferences.
- Excellent communication, interpersonal, and negotiation abilities.
- Maintain accurate records and documentation in line with organisational policies and government funded guidelines.
- A collaborative approach to working as part of a team.
- Knowledge of Commonwealth Support Programme (CHSP) is highly valued.
- Demonstrated experience in assessment, referral, and coordination of human services.
- An unrestricted driver's license.
