Job Overview:

We are seeking an experienced support coordinator to join our team in a part-time capacity. The successful candidate will empower individuals with disabilities to engage with community and mainstream services, aligning with their NDIS plans.

The key responsibilities of this role include:

  • Developing and maintaining relationships with service providers to achieve participant goals.
  • Monitoring plan outcomes and managing expenditure effectively.
  • Coordinating and implementing strategies to maximize participants' NDIS plan goals.

To be successful, you must have at least 2 years of experience in NDIS Support Coordination or a Community Case Management role. You should be passionate about the NDIS and supporting participants.
