Assessment and Care Coordination Opportunity

This exciting opportunity is available for a Coordinator to join our team in Veterans' Home Care. As a key member of our team, you will play a vital role in ensuring that eligible veterans receive high-quality support.

As a Coordinator, your primary responsibilities will include conducting assessments and reviews, formulating care plans based on sound clinical rationale, and providing service coordination to support vulnerable or at-risk clients.

You will work closely with a trusted team of professionals who share your passion for delivering exceptional customer service. Together, you will provide empathetic, supportive, and comprehensive care to veterans and their families.

Key Responsibilities:

  • Conduct assessments and reviews to formulate care plans
  • Provide service coordination to support vulnerable or at-risk clients
  • Work collaboratively with healthcare professionals, community organizations, and government agencies to achieve best outcomes for clients
  • Advocate for clients and their families, and facilitate referrals to appropriate care agencies where necessary

Requirements:

To be successful in this role, you will require:

  • Tertiary qualifications in a health or social services-related discipline or equivalent experience in the aged, community, or healthcare industry
  • Experience in assessment, care planning, and service coordination
  • Excellent communication and interpersonal skills, with a professional and courteous manner
  • A collaborative 'can-do' approach, with a focus on achieving best outcomes for clients
  • Proficiency in Microsoft Office programs and other relevant technology
