Planned Surgery Patient Care Coordinator

We are seeking an experienced Registered Nurse to join our Planned Surgery and Endoscopy Access team as a Part Time, Fixed term Planned Surgery Liaison Nurse for the Endoscopy Unit.
About the Role:- The role focuses on optimizing patient preparation and streamlining surgical pathways to ensure efficient and safe delivery of planned surgical procedures.
- You will play a vital role in supporting patients throughout their surgical journey from initial referral to surgery.
- You will collaborate with multidisciplinary teams conduct comprehensive patient assessments proactively identify and mitigate potential delays ensuring all patient care aligns with the Department of Health's Planned Surgery Access Policy.
- Current AHPRA registration as a Registered Nurse.
- Relevant post-graduate qualification or extensive relevant clinical experience.
- Demonstrated skills in acute nursing care.
- Effective management communication and interpersonal skills including conflict resolution.
- Comprehensive patient assessment and readiness for surgery evaluation.
- Collaboration with Endoscopy unit and multidisciplinary teams to support patient optimization.
- Maintaining compliance with the Planned Surgery Access Policy & Elective Surgery Information System (ESIS).
