Empowering Lives: Disability Support Coordinator

This is an exciting opportunity to join our team as a Support Coordinator where you will play a vital role in empowering individuals with disabilities to achieve their goals and live fulfilling lives.
As a Support Coordinator, you will work closely with participants, their families, and carers to understand their needs and develop tailored plans that promote independence, choice, and control.
Key responsibilities of the role include:
- Coordinating daily operations of support coordination services
- Contributing to participant assessment care planning, responding to referrals and reviews in accordance with policies and procedures
- Advocating internally and externally to protect the rights of participants and promote their interests
- Ensuring work practices and processes are carried out in accordance with quality guidelines and notifying the director of any quality issues
- Contributing to a culture of continuous improvement that reviews indicators of participant satisfaction and implements actions to achieve results
The ideal candidate will possess a working knowledge of the NDIS and disability space, as well as relevant qualifications and experience. They will be able to communicate effectively with people from diverse backgrounds and manage competing priorities in a fast-paced environment.
