Job Overview:

The Compassionate Care Specialist provides support and guidance for patients who need additional assistance as they navigate the healthcare system. This includes conducting a comprehensive assessment of patient and family needs, developing individualized care plans, and facilitating the admission process to our home care services.

Key Responsibilities:

  • Conduct initial assessments of patient and family needs to determine required home care services
  • Develop and implement tailored care plans that address each patient's unique needs and goals
  • Foster open communication with patients and families to establish shared goals and objectives
  • Aid in the admission process by performing evaluations, assessing psychosocial status, and identifying factors affecting treatment plans
  • Monitor and record changes in patient emotional and social factors impacting their illness and care requirements
  • Collaborate with attending physicians on treatment plan adjustments as needed
  • Maintain accurate, written clinical records, including initial evaluations, care plans, and daily notes

Requirements:

  • Bachelor's Degree from an accredited program or higher in Social Work or related field
  • Certification from a recognized professional organization (e.g., Academy of Certified Social Workers)
  • Minimum 1 year of experience in a healthcare setting, preferably with a focus on home care or social work
  • Criminal background clearance
  • Current CPR certification
