Job Opportunity

The RN Care Manager plays a pivotal role in driving improved health outcomes through a population-based approach. This position assesses, plans, educates, monitors, evaluates, and coordinates services for patient populations and their families to ensure quality, continuity, and optimal utilization of healthcare resources throughout the care continuum.

Key Responsibilities:
  • Facilitate resource coordination for maximum health outcomes, patient satisfaction, and financial optimization.
  • Assist patients, families, and caregivers in securing necessary medications, equipment, and community resources to promote health and overall wellness.
  • Collaborate with other healthcare team members to coordinate preventive care and chronic disease management services.
Requirements:
  • Active registration as a licensed registered nurse in the State of Ohio.
  • Three years of clinical experience in care coordination, including strong expertise in managing diabetic populations and other chronic diseases.
  • Familiarity with Epic electronic medical records software is preferred.
Preferred Qualifications:
  • One year of experience in Care Management.
  • Additional relevant experience or preparation may be beneficial for specialty care management.
