Empowering Lives: A Support Coordination Role

Join a forward-thinking organisation in a rewarding role that makes a real difference in people's lives. As a Specialist Support Coordinator, you will provide comprehensive support coordination to participants with complex needs, behaviours of concern, medical needs, or other high-risk issues.
CareChoice is passionate about supporting individuals with disabilities and empowering them to make informed choices. We are seeking a highly skilled and compassionate professional to join our team in this hybrid role.
Responsibilities:- Assist participants at the specialist level to implement their NDIS plans by maximising choice and control and engaging high-quality service providers.
- Contribute reporting for and participate in review processes for the NDIS participants as requested or required.
- Manage and coordinate any complex care team requirements for the participant where they have specialist support coordination.
- Maintain accurate records, relevant reporting, and communication in relation to the participant and their care team.
- Evaluate potential risks associated with program provision and report these to management.
This role requires someone who is highly organised, able to work independently, and has excellent communication skills. You will be working closely with participants, support workers, and other stakeholders to ensure the highest quality of care and support.
