Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Care Manager. As a Care Manager, you will be responsible for providing high-quality care and support to our clients in their own homes. This is a rewarding opportunity to make a real difference in people's lives and be part of a dynamic and supportive team.

Key Responsibilities
  • Develop and implement personalized care plans for clients, in collaboration with our care team and other healthcare professionals.
  • Provide hands-on care and support to clients, including medication management, wound care, and other nursing tasks.
  • Conduct regular assessments and evaluations to ensure the highest level of care and support.
  • Collaborate with our care team to ensure seamless care delivery and excellent client outcomes.
  • Participate in ongoing training and professional development to stay up-to-date with best practices and industry developments.
Requirements
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Appropriate qualifications and experience in the aged care sector, with a focus on home and community care.
  • Australian Federal Police Clearance Certificate.
  • Driver's License and comprehensively insured dependable private vehicle.
  • Proven ability to work independently and as part of a team, with excellent communication and interpersonal skills.

  • Strong time management and organizational skills.
  • Ability to create and promote a positive and supportive culture within the team.
  • Computer literacy and proficiency in care management software.
