The RoleWe are seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for managing client relationships, delivering excellent customer service, and implementing client care initiatives. This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with various teams to ensure high-quality service delivery.
Key Responsibilities- Represent the company as a leading healthcare provider, promoting our services and reputation to key stakeholders.
- Ensure a high standard of conduct and work performance, upholding the company's values and reputation.
- Actively promote home care services to meet growth targets, developing and implementing strategies to achieve business objectives.
- Deliver on agreed KPIs and targets, ensuring timely and effective service delivery.
- Manage a caseload of clients, delivering excellent client experiences and ensuring high-quality care outcomes.
- Develop care plans that support the delivery of quality outcomes for clients, in line with their funding and care needs.
- Ensure all client documentation and case notes are up to date, complete, and of a high standard.
- Support the business development and onboarding function, including the assessment and suitability to provide services in line with company policies and procedures.
- Relevant tertiary qualifications or experience in healthcare or a related field.
- Strong understanding of aged care and/or other health-related services.
- Experience and success in working in a customer-focused service business.
