The Role

We're seeking a highly skilled Care Manager to join our team in Sydney. As a Care Manager, you'll be responsible for managing client relationships, delivering excellent customer service, and implementing client care initiatives. This role requires strong communication skills, attention to detail, and the ability to work collaboratively with various teams.

Key Responsibilities
  • Client Relationship Management: Develop and maintain strong relationships with clients, ensuring their needs are met and exceeded.
  • Service Delivery: Ensure that all service delivery and compliance reporting requirements are met, maintaining high standards of quality and excellence.
  • Care Planning: Develop and implement care plans that support the delivery of quality outcomes for clients, in line with their funding and needs.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date client documentation and case notes, adhering to company policies and procedures.
  • Business Development: Support the business development and onboarding function, including the assessment and suitability to provide services in line with company policies and procedures.
Requirements
  • Relevant Qualifications: Relevant tertiary qualifications or experience in the healthcare industry or similar.
  • Strong Understanding: Strong understanding of aged care and/or other health-related services.
  • Customer-Focused Experience: Experience and success in working in a customer-focused service business.
Desired Qualifications
  • Enrolled Nurse - AHPRA: Enrolled Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Registered Nurse - AHPRA: Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA).
