Job Title: Clinical Care Specialist - Community Nursing

About the Role:


As a Clinical Care Specialist - Community Nursing, you will play a vital role in delivering high-quality, patient-centered care to individuals in regional, rural, and remote Australia. You will work independently to assess, develop, and evaluate nursing care for clients with diverse needs, collaborating closely with Allied Health clinicians to achieve optimal health outcomes.


Key Responsibilities:



  • Providing health promotion and education to clients and carers, promoting independence and self-management of health needs
  • Delivering award-winning Wellness for Independence programs, promoting holistic assessment skills and practical expertise
  • Monitoring and promoting health outcomes via Telehealth, working collaboratively with clients and healthcare teams
  • Providing direct care services, including wound care, diabetes management, medication management, stoma care, and catheter care, with a focus on patient-centered care
  • Assessing, developing, and evaluating nursing care for clients with diverse needs, working collaboratively with interdisciplinary teams

Requirements:



  • Division One Registered Nurse with the Australian Health Practitioners' Regulation Authority (AHPRA)
  • Experience working in community, acute, and aged care settings, with a focus on primary health care and consumer-directed care
  • Proficient in the use of electronic technology, including smart phones, tablets, laptops, email, internet, and client data management systems
  • Strong communication and interpersonal skills, with the ability to work independently and collaboratively as part of a team
  • Ability to work systematically and accurately, with a focus on patient safety and well-being
  • Current driver's license and ability to travel for client care
