Job Overview

This role empowers the existing system to deliver integrated and wrap-around services to vulnerable families in Western Sydney.

Key Responsibilities

  • Provide specialist information and assistance to key service providers to identify vulnerable families and their needs, support access and coordination of quality services between agencies to families, and ensure there is a coordinated shared family plan for ongoing support to families.
  • Develop and participate in service development and continuous quality improvement to ensure the project meets key deliverables.
  • Work within a multiagency team to develop and implement service navigation pathways with key stakeholders, develop capability uplift in service providers, and develop a systemic approach to building sustainability within the service sector.

Required Skills and Qualifications

  • Tertiary qualification in a relevant allied health related discipline and registration as required and demonstrated post-graduate experience in the delivery of services and/or case management to vulnerable children and their families with specialist knowledge in disability.
  • Demonstrated understanding of the service system which supports children and their families, and the interconnectivity between government and non-government agencies involved in supporting families.
  • Demonstrated ability to build strong and effective relationships with both internal and external stakeholders and experience in the development of interagency collaborations and innovative ways of working.
