Job Title: Mental Health Specialist - Huntington's Disease Care
About the RoleThis is an opportunity to make a meaningful difference in the lives of individuals and families impacted by Huntington's Disease.
As a specialist in mental health care, you will be responsible for delivering case management and clinical support within a multidisciplinary team.
You will coordinate care across community, inpatient, and outpatient settings, provide education and therapeutic interventions, and collaborate with health professionals and support services to ensure holistic, person-centred care.
Key Responsibilities- Deliver specialised case management and clinical support to individuals and families impacted by Huntington's Disease
- Coordinate care across community, inpatient, and outpatient settings
- Provide education and therapeutic interventions
- Collaborate with health professionals and support services
To succeed in this role, you will need:
- A minimum of two years' post-graduate employment in a clinical area directly applicable to the OPMHS and/or Huntington's Disease Service area
- Demonstrated expertise in the theoretical and practical application of a range of therapeutic interventions relevant to OPMHS and/or Huntington's Disease Service setting
- Experience and demonstrated knowledge and skills in relation to liaison and consultation with other services and agencies
