About the Role:As a Community Mental Health Specialist, you will work closely with a multidisciplinary team to deliver integrated, evidence-based care across inpatient, outpatient, and community settings.
Key Responsibilities:- Provide specialist case management and clinical support to individuals and families impacted by Huntington's Disease
- Coordinate care across different settings to ensure holistic, person-centred care
- Offer education and therapeutic interventions tailored to individual needs
- Collaborate with healthcare professionals and support services to achieve optimal outcomes
- A minimum of two years' post-graduate experience in a clinical area relevant to this role
- Demonstrated expertise in mental health assessment and intervention, including ability to provide appropriate assessments
- Specialized knowledge in theoretical and practical application of therapeutic interventions relevant to Huntington's Disease
