About the Position

The Community Geriatrics service provides short-term case management to prevent hospital admissions through the delivery of care in the home environment. As a successful applicant, you will be responsible for providing leadership to the team, including supporting recruitment, performance management and development, rostering, allocating tasks, and other human resource management.

  • Providing services to community-dwelling patients via multiple modalities, including face-to-face and telehealth consultations.
  • Applying clinical expertise to assess clients, select and implement different therapeutic interventions and/or support programs, and evaluate patient/client progress.
  • Linking clients and carers with relevant community resources and services.
  • Communicating effectively with the client's family, carers and other service providers.
