Are you passionate about helping people with disabilities reach their full potential? We have an exciting opportunity for a Employment Pathways Practitioner to join our team in Ballarat, Victoria.

About the Role

We are looking for a skilled and dedicated individual to provide employment assistance to participants with disabilities. As an Employment Pathways Practitioner, you will work closely with our team to tailor innovative supports and help individuals develop their capacity, build their skills, and work towards their employment-related goals.

Key Responsibilities
  • Provide customized employment assistance to participants with disabilities
  • Conduct comprehensive discovery processes to identify participant needs and goals
  • Mentor participants within open employment workplaces
  • Coordinate job skills development and assessments
  • Manage and supervise employment preparation tasks
  • Oversee job searching and job creation
  • Facilitate post-placement supports once a job has been secured
Requirements

To be successful in this role, you will need:

  • Cert IV in disability, community services, employment services or a related field
  • Current NDIS Worker Screening Check or willingness to obtain
  • Ability to transport participants in your own vehicle (current license, car, and comprehensive insurance required)
  • Sound understanding and experience in delivering person-centered supports
  • Experience working in the NDIS environment
