Based at Inverell Community Health, you will provide comprehensive support to individuals with dementia while working alongside an enthusiastic and collaborative team.
This is a permanent part-time position (20 hours per week) with Requisition IDREQ600524and Applications Close onSunday 7 September.
Remuneration:$36.39 - $51.09per hour + Super + Salary Packaging.

Responsibilities
Organise a Clinic for Dementia and Aged Care alongside a Rehabilitation Specialist
Provide vital support through home visits
Contribute to person- and family-centred care within a multidisciplinary team
About You / Qualifications
Current registration with AHPRA as a Registered Nurse
Proven clinical experience in dementia, delirium, or aged care case management
Excellent communication, assessment, and time management skills
Ability to work autonomously and as part of a multidisciplinary team
Genuine interest in dementia care and supporting carers and community teams; culturally aware and empathetic
