Role Overview:

Complex Care Specialist

This team delivers vital support services and care to participants of the NDIS, providing a unique opportunity to make a meaningful impact in the lives of vulnerable individuals.

The Complex Care Specialist will develop, implement, and manage individualized care plans, working closely with clients to identify and address their complex needs. This role requires exceptional interpersonal skills, emotional intelligence, and a non-judgmental approach to supporting clients with diverse backgrounds and experiences.

Key Responsibilities:

  • Developing and delivering comprehensive care plans that cater to the unique needs of each client.
  • Providing high-quality support services, including daily living assistance, behavioral support, and coordination of external services.
  • Working collaboratively with multidisciplinary teams, including healthcare professionals, social workers, and other stakeholders to ensure seamless service delivery.
