Job Description

Making a meaningful difference in the lives of clients is at the heart of this role. As a Support Worker, you will be assisting with activities of daily living and providing support services such as Domestic Assistance, Social Support, Specialist Children and Adult Services, Respite and Personal Care at their home and in the community.

About the Role

You will provide individualised and respectful personal care assistance to our Residents in Aged Care, ensuring they live the life they choose and exercise choice and independence. This includes supporting residents to maintain their physical, emotional and lifestyle needs and wants.

Key Responsibilities
  • Support residents in achieving their goals and aspirations while promoting their autonomy and self-reliance.
  • Contribute to the assessment and observation of resident needs, implementing clinical and care interventions as delegated by the Registered Nurse and documented in the resident's plan of care.
  • Assist residents with their personal care and hygiene needs, encouraging their independence and dignity.
  • Document and report progress notes, including the evaluation of interventions and any changes to resident care plans.
Required Skills and Qualifications

This role requires a Certificate III in Aged Care, Home and Community Care, Disability or Individual Support (desired). You must also hold a current and valid Driver's Licence, have access to a reliable motor vehicle and a smart mobile phone.
