Job Description:

We are seeking a skilled and compassionate Home Care Worker to join our team. As a key member of our team, you will provide person-centered care and support to clients in their own homes, enabling them to maintain their independence and quality of life.

The role involves traveling between client homes, assisting with daily living activities, and providing emotional support. You will work closely with our Care Coordinators to understand the needs of each client and develop tailored care plans.

Key Responsibilities:

  • Assist clients with daily living activities, such as personal care, meal preparation, and light housekeeping
  • Provide social support, including accompanying clients to appointments and engaging in recreational activities
  • Monitor client well-being and report any changes or concerns to the Care Coordinator
  • Participate in staff meetings and training sessions to enhance your skills and knowledge

Requirements:

  • Australian Federal Police Clearance
  • Current CPR/First Aid Certificate
  • NDIS Workers Screening Check (or willingness to obtain)
  • Current Australian Driver's Licence
  • Reliable, registered, and insured vehicle
  • Smartphone with internet access
