Care Leader for Older Persons

The Nurse Practitioner (NP) or Transitional Nurse Practitioner (TNP) will address service gaps and improve health outcomes for older persons by providing timely access to specialist aged healthcare.
- Responsibilities include preventing deterioration, reducing hospital presentations and admissions, while collaborating with other healthcare team members.
- The primary metaspecialty is 'Aged Care', with supported development of your Scope of Practice (ScOP) into 'Emergency and Acute Care', 'Mental Health', and 'Chronic and Complex Care'.
The Transitional Nurse Practitioner is appointed for a provisional period of up to 3 years to work towards endorsement as a Nurse Practitioner, developing expertise in dynamic Models of Care (MoC).
Key Responsibilities:
- Improve health outcomes for older persons by providing timely access to specialist aged healthcare.
- Collaborate with other healthcare team members to achieve shared goals.
- Develop your Scope of Practice (ScOP) into 'Emergency and Acute Care', 'Mental Health', and 'Chronic and Complex Care'.
