Job Title

We are seeking a skilled Nurse Practitioner to enhance access to healthcare for older persons.

The successful candidate will work across various departmental locations, including Inpatient Units, Outpatient Services, In-reach, Outreach, Community, and Emergency. They will provide timely access to specialist aged healthcare, prevent deterioration, reduce hospital presentations and admissions, while collaborating with other healthcare team members.

The primary metaspecialty is 'Aged Care', with supported development of your Scope of Practice into 'Emergency and Acute Care', 'Mental Health', and 'Chronic and Complex Care'. The role requires expertise in Aged Care, Emergency and Acute Care, Mental Health, and Chronic and Complex Care.

About the Role

This is a unique opportunity for a Registered Nurse to develop their skills and expertise as a Transitional Nurse Practitioner. You will work towards endorsement as a Nurse Practitioner during a provisional period of up to 3 years.

Key Responsibilities
  • To improve health outcomes for older persons by providing timely access to specialist aged healthcare.
  • To prevent deterioration, reduce hospital presentations and admissions, while collaborating with other healthcare team members.
  • To develop expertise in Aged Care, Emergency and Acute Care, Mental Health, and Chronic and Complex Care.
  • To work across various departmental locations, including Inpatient Units, Outpatient Services, In-reach, Outreach, Community, and Emergency.
Requirements
  • Current registration as a Registered Nurse and Eligible for NP endorsement with AHPRA.
  • Postgraduate qualifications in Nursing or a relevant field.
  • Minimum 2 years' experience in clinical practice.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and collaboratively as part of a multidisciplinary team.
