Patient Care Coordinator

At Calvary Care, we are seeking a dedicated Patient Care Coordinator to join our team. The ideal candidate will be a Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA), possessing excellent communication and interpersonal skills.
This role offers a rewarding opportunity to make a meaningful difference in the lives of our residents, working collaboratively with the Home Manager to foster an environment where they feel valued and respected.
About the Role- We are offering a permanent Full-Time opportunity with a competitive salary package that includes NFP salary packaging benefits, discounted health insurance, and gym memberships.
- The successful candidate will have a proven ability in quality management using a team-based approach through mentoring and staff development, as well as a working knowledge of WH&S and Infection Control.
- Your duties will include managing and mentoring RNs and other clinical staff, ensuring residents receive the highest quality of care and service, and providing expert advice and guidance on clinical matters.
- Collaborate with the Home Manager to develop and implement policies and procedures that promote best practice within the facility.
- Liaise with external stakeholders and health professionals to ensure compliance with regulatory requirements.
- Develop and maintain effective relationships with residents, families, and healthcare teams to deliver exceptional care outcomes.

- Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- An effective listener with the ability to communicate at all levels both orally and in writing with excellent presentation skills.
- Understanding of and ability to manage human resource issues with effective negotiation skills.
- Ability to understand and use information technology and programs and to learn new skills as required.
- An inherent curiosity and a personal value of wanting to do better all the time, a change leader.
- Working knowledge of WH&S and Infection Control.
The estimated annual salary for this role is $AUD 110,000 - $140,000 depending on experience and qualifications.
