Role Summary

This key role involves the management of patient admissions for a Hospital in the Home service. As part of a care coordination model, you will work closely with multidisciplinary teams to ensure efficient and effective patient management and discharge planning.

Main Responsibilities
  • You will be responsible for promoting the HITH service within your organisation and externally.
  • Your efficiency and innovative ways of working with electronic medical records will be highly valued.
  • As a leader, you will guide patient care, treatment and management.
  • You will also provide leadership and direction to nurse clinicians.
Qualifications and Experience Required
  • You must be registered as a Registered Nurse with AHPRA.
  • Experience in acute and/or ambulatory/community settings is desirable.
