Clinical Care and Support Specialist

This is an exciting opportunity to make a meaningful impact in the lives of residents. As a Clinical Care Coordinator, you will play a pivotal role in delivering high-quality clinical care and coordinating with the care team and health professionals.
Key Responsibilities:- Undertake or supervise others to undertake clinical care for individual residents as required, including complex wound care, pain management, behaviour management, end-of-life care, and other clinical procedures as necessary.
- Liaise with residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive high-level clinical care.
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) via AHPRA.
- Recent experience in caring for high-care and dementia residents.
- Knowledge of wound care, palliative care, behaviour management, clinical assessments, and care plans.
