About the Role

This is a part-time, fixed-term opportunity to join our dynamic team as a Palliative Care Clinical Nurse Specialist. As a valued member of our Wodonga Community Palliative Care Team, you will play a crucial role in undertaking triage and assessments of individuals with life-limiting illnesses in the local community.

Key Responsibilities

  • Undertake triage and assessments of individuals with life-limiting illnesses in the community
  • Work collaboratively with the Wodonga Community Palliative Care Team to provide high-quality patient care
  • Develop and implement individualized care plans to meet the unique needs of patients and their families
  • Provide education and support to patients, families, and healthcare professionals on palliative care principles and practices
  • Contribute to the development and implementation of palliative care policies and procedures

Requirements

  • Current AHPRA registration
  • Current Working with Children Check
  • Excellent communication and problem-solving skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong knowledge of palliative care principles and practices
  • Ability to work in a fast-paced environment and prioritize tasks effectively
