Care Manager

About the Role:
We are seeking a highly skilled and experienced Care Manager to lead our clinical team in a short-term 3-month contract in Wodonga. As a key member of our team, you will be responsible for overseeing the delivery of person-centred care for our residents. Your primary responsibilities will include ongoing assessment, care planning, evaluation, and ensuring clinical governance to enhance the quality of life for our residents.
Key Responsibilities:
- Supervise the day-to-day clinical operations of the care home, ensuring compliance with regulatory requirements and best practices.
- Lead and support the care home clinical team through coaching, mentoring, and performance management, promoting a culture of excellence and continuous improvement.
- Manage compliance with ACFI assessments, ensuring accurate and timely completion of assessments and care plans.
- Facilitate person-centred care planning and evaluation for residents, including family and carer involvement, to ensure individualized and holistic care.
- Conduct clinical governance audits to identify areas for improvement and implement changes to enhance the quality of care.
- Oversee medication management in line with resident needs and legislative requirements, ensuring safe and effective medication administration.
- Coordinate with allied health professionals, medical practitioners, and other services to optimize resident care and outcomes.
Requirements:
- Current registration with AHPRA is essential.
- Bachelor of Nursing or Equivalent qualification is essential.
- Postgraduate qualification in aged care is highly regarded.
- Minimum of 30 beds management experience.
- Proven track record in delivering quality clinical care within aged care or acute settings.
- Strong clinical assessment and care planning skills.
- Excellent communication, leadership, and relationship management abilities.
- Understanding of Resident Rights, Aged Care Accreditation Standards, and OHS guidelines.
