Key Responsibilities- Undertake triage and assessments of patients with life-limiting illnesses
- Work collaboratively with the healthcare team to develop and implement individualized care plans
- Provide education and support to patients and their families
- Contribute to the development and implementation of palliative care services
- Current AHPRA registration
- Current Working with Children Check
