Population Health Specialist

We are seeking a highly skilled and motivated individual to join our team as a Population Health Specialist. This role will contribute to the development and implementation of projects aimed at improving population health and wellbeing outcomes.
- This includes assisting in research, analysis and implementation of projects based on defined outcomes and outputs.
- Supporting the development and delivery of projects or sub-projects and their successful delivery and completion within time, quality and budget parameters.
- Contributing to the monitoring and evaluation of projects to ensure departmental objectives are met.
The ideal candidate will have strong analytical and problem-solving skills, with the ability to work effectively in a team environment.
The key responsibilities of this role include:
- Developing and implementing agreed work plans, service monitoring, quality improvement and innovative approaches to prevention and population health.
- Collaborating with funded organisations, local governments and other partners to achieve policy implementation and statewide policy processes.
- Providing administrative and business system support across the public health teams and supporting Emergency Management surge functions.
Maintaining accurate records, working in accordance with legislative requirements and occupational health and safety policies is essential for this role.
