We are seeking a skilled and compassionate Community Care Specialist to join our team.
This is an exciting opportunity for the right individual to make a positive impact in the lives of older people and their families.
About the Role:
The Community Care Specialist will provide personalized support and care to clients within their own homes, promoting independence and enabling them to remain engaged with their community.
The ideal candidate will possess excellent communication skills, be able to work autonomously and have high-level time management skills.
Key Responsibilities:
- Providing Social Support: Offer emotional support, companionship and assistance with daily tasks to enhance the quality of life for clients.
- Personalized Care: Deliver tailored support and care plans that meet the unique needs and goals of each client.
- Medication Management: Assist clients with medication administration, ensuring adherence to prescribed treatment plans.
- Meal Preparation and Domestic Assistance: Prepare healthy meals and assist with light domestic duties, promoting a safe and comfortable living environment.
Requirements:
- Certification: Hold a Cert III qualification in Aged Care or equivalent qualifications.
- First Aid and CPR Certification: Possess current First Aid and CPR certification.
- Driving License: Hold a current Open Drivers Licence and a reliable vehicle with comprehensive insurance.
- National Police Certificate: Obtain a National Police Certificate or willingness to obtain.
