Key Responsibilities:
  • Deliver counseling and case management services to patients/families with complex needs.
  • Work with diverse client groups to provide culturally sensitive support.
  • Manage patient cases and develop treatment plans in collaboration with multidisciplinary teams.
Requirements:
  • Degree in Social Work with eligibility for membership of the Australian Association of Social Work.
  • Experience in social work or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
