
Senior Care Professional

We are seeking a highly skilled and compassionate individual to join our team as a Care Coordinator. As a key member of our care management team, you will be responsible for ensuring the delivery of high-quality care services to our clients.
Your primary focus will be on conducting thorough client assessments and care planning to meet the unique needs of each individual. You will also be responsible for managing client compliance, intake processing, and reporting. Additionally, you will play a crucial role in coordinating the provision of quality person-centered care according to client needs, service plans, and available funding.
Responsibilities:- Conduct client care assessments and develop personalized care plans
- Manage client compliance and caseloads
- Process intakes, conduct clinical care planning, and provide regular reporting
- Coordinate the delivery of quality person-centered care
- Perform general administrative tasks
- Participate in on-call rotations with the care management team
- Provide exceptional customer service via phone and in-person inquiries
To be successful in this role, you will require:
- A minimum of Certificate 3 in Aged Care/Individual Support
- A current driver's license
In addition to the qualifications and requirements listed above, we are seeking candidates with:
- Previous experience in aged care, NDIS, or mental health care coordination
- Knowledge of My Aged Care, Home Care Package, and NDIS
- Enrolled nurse or registered nurse qualifications
- Experience in assessing and planning care needs within the community
- Valid driver's license
- High level of computer literacy, including Microsoft Office
- An outgoing personality with high energy levels, excellent communication skills, and a personable telephone manner
- Ability to problem-solve and think critically under pressure
- Possess a 'can-do' positive attitude and work well in a team environment
- Ability to multitask with a strong attention to detail while remaining calm and focused
- Excellent networking and consultation skills to develop new referral sources and relationships with external organizations
- Exceptional customer service skills with outstanding verbal and written communication skills
- Sound knowledge and understanding of home care services and guidelines
