Public Health Care Specialist

We are seeking an experienced and skilled Public Health Nurse to provide high-quality patient care and support in a dynamic organization.
This role will play a vital part in promoting health education, disease prevention, and management. You will be responsible for assessing patients' needs, developing individualized care plans, and collaborating with multidisciplinary teams to ensure comprehensive care.
Key Responsibilities:- Assess patients' physical and mental health needs, develop, and implement individualized care plans.
- Provide health education, promote healthy behaviors, and encourage self-management.
- Collaborate with multidisciplinary teams, including physicians, social workers, and other healthcare professionals.
- Conduct home visits, community outreach, and other activities to engage patients and their families.
Requirements:
- Registered Nurse (RN) license and current certification as a Public Health Nurse.
- Minimum 2 years of experience in public health nursing, preferably in a hospital or community setting.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Proficiency in electronic health records (EHRs), Microsoft Office, and other software applications.
