Job Opportunity: Community Support Coordinator

This role is responsible for leading a team to deliver quality services supporting clients in their own home and connected to their local community.

The coordinator will facilitate strong relationships with each client and their support circle, overseeing service coordination and navigating individual needs based on client requirements.

Key skills required include supervising teams through ambiguity and change, understanding client needs, and applying critical thinking to achieve positive outcomes.

Requirements
  • Tertiary qualification in Aged Care, Health, Business, or equivalent years of experience
  • Two or more years' experience in team supervision and nurturing during change
  • Experience in Aged Care and/or Health environment
  • Significant understanding of client needs for positive service delivery
  • Critical thinking skills with the client at centre
  • Strong working relationships with internal and external stakeholders
  • Passion for social change and contributing to vulnerable community members
  • Proficient in computer applications with willingness to learn new systems
