Expert Support Coordinator

We are seeking an experienced Support Coordinator to lead our team with expertise and drive results.
About the Position:
- Coordinate high-quality support for participants with complex needs.
- Develop strong relationships with participants, their families, and service providers.
- Contribute to continuous improvement and best practices.
- Maintain compliance with NDIS guidelines and documentation standards.
Key Responsibilities:
- Support participants with complex needs to navigate the NDIS, build independence, and maximize plan benefits.
- Design and implement tailored support plans that meet participant requirements.
- Record and maintain accurate and up-to-date records of participant interactions and progress.
Required Qualifications and Skills:
- Proven experience in Support Coordination, particularly with complex needs.
- Thorough understanding of the NDIS framework.
- Excellent organizational and interpersonal skills.
- A solutions-focused mindset with a client-centered approach.
- Relevant qualifications in Social Work, Community Services, or similar fields.
- NDIS Worker Screening, WWCC, First Aid, and Driver's License.
