About the Role

We are seeking a skilled Mental Health Worker to join our team at Centacare, a leading provider of community services in South Australia. As a Mental Health Worker, you will play a vital role in supporting individuals experiencing mental health challenges and distress across various regions.

Key Responsibilities
  • Provide person-centred psychosocial support to clients, including goal setting, exit planning, and transition to NDIS services.
  • Work collaboratively with clients, their families, and carers to develop and implement individualised support plans.
  • Conduct assessments and develop case plans to address client needs and goals.
  • Provide crisis intervention and support to clients in times of need.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery and client outcomes.
Requirements
  • Certificate IV level qualification in Mental Health (non-clinical) or equivalent qualification.
  • Experience working directly with people with mental health issues, complex needs, and with their families and carers.
  • Proven ability to work autonomously and as a member of a team.
  • Demonstrated ability to plan and prioritise to meet client service delivery requirements.
  • Demonstrated experience in documenting client case notes and working with electronic data bases.
  • Demonstrated understanding of available community services, networks, and supports.
  • Demonstrated understanding of the NDIS eligibility, application, and transition processes.
  • Great interpersonal and listening skills to build strong relationships with key stakeholders.
  • Excellent customer service skills.
  • Demonstrated experience working with the recovery framework and taking a trauma-informed approach to practice.
  • Ability to make sound decisions under pressure and de-escalate crises.
  • Valid Department of Human Services (DHS) Working with Children Check.
