The role of a Clinical Partner involves assessing clients' needs, developing and reviewing personalized care plans, and delivering high-quality clinical care with compassion and expertise. In partnership with clients and stakeholders, the Clinical Partner provides guidance to care staff to ensure safe and continuous delivery of care.

To succeed in this role, you will need:

  • A strong commitment to our mission and values
  • A Registered Nurse qualification and current registration with AHPRA as a Registered Nurse (Division 1, General)
  • Experience in delivering client-focused services in aged and community/home care settings
  • Detailed knowledge of Aged and Home Care Reforms and Aged Care Quality Standards
  • Excellent communication and collaboration skills, with a focus on advocacy and person-centered care
  • An ability to work effectively within a multidisciplinary team and independently across multiple sites
