As a Nurse Navigator, you will provide advanced clinical leadership and coordinate complex care for patients across the Logan Hospital catchment.
About the RoleThis position focuses on improving patient outcomes, streamlining services, and creating a more integrated healthcare journey for people with disability and their families.
- Provide advanced nursing leadership and expertise in disability care.
- Coordinate patient-centred care across primary, secondary, and tertiary providers.
You will be responsible for:
- Engaging patients and families in self-management, advocacy, and informed decision-making.
- Driving quality improvement, research, and system innovation to improve outcomes.
A highly skilled and compassionate nurse who thrives in complex environments and is committed to advocating for people with disability.
- Current AHPRA registration as a Registered Nurse with extensive clinical experience.
- Advanced knowledge of disability care, including NDIS, guardianship, and restrictive practices.
- Proven leadership, problem-solving, and change management skills.
