The Chronic Disease Management Health Professional plays a pivotal role in supporting GPs and patients in managing chronic conditions, focusing on preventative health to keep patients out of hospital.
This experienced professional will be responsible for preparing care plans and team care arrangements, empowering patients to take ownership of their health, coaching patients on health behavior change principles, facilitating access to ongoing care from a multidisciplinary team, assisting in early identification of chronic diseases through health assessments, working with GPs to promote preventative healthcare, increasing first-time care plan recipients, reviewing care plans regularly, and managing administrative tasks related to CDM appointments.
Key Responsibilities:- Develop comprehensive care plans and team care arrangements for patient disease management.
- Empower patients to assume responsibility for their health.
- Coach patients on effective health behavior change strategies.
- Facilitate ongoing access to care from a multidisciplinary team.
- Assist in the early identification of chronic diseases through health assessments.
- Collaborate with GPs to promote proactive healthcare approaches.
- Increase the number of first-time care plan recipients.
- Regularly review and update care plans as necessary.
- Manage administrative tasks associated with CDM appointments.
- AHPRA registered Enrolled or Registered Nurse.
- Experience in Chronic Disease Management and health assessments.
- Knowledge of Health Assessment is advantageous.
- Strong organizational and time management skills.
- Excellent communication and customer service skills.
- Passion for patient care and a personable approach.
