The Role

We are seeking a Full Time experienced Chronic Disease Management (CDM) Nurse to join our Murrumba Downs practice. You will support GPs and patients in managing chronic conditions, focusing on preventative health to keep patients out of hospital.

The Tasks
  1. Prepare Care Plans and Team Care Arrangements for patient disease management.
  2. Empower patients to take ownership of their health.
  3. Coach patients on health behavior change principles.
  4. Facilitate access to ongoing care from a multidisciplinary team.
  5. Assist in early identification of chronic diseases through health assessments.
  6. Work with GPs to promote preventative healthcare.
  7. Increase first-time care plan recipients.
  8. Review care plans regularly.
  9. Manage administrative tasks related to CDM appointments.
The Successful Candidate
  • AHPRA registered Enrolled or Registered Nurse.
  • Experience in Chronic Disease Management and health assessments.
  • Knowledge of Health Assessment is advantageous.
  • Strong organizational and time management skills.
  • Excellent communication and customer service skills.
  • Passion for patient care and a personable approach.
